Family Care Community Partnership

Girl with a baloon animalThe Northern Rhode Island Family Care Community Partnership (FCCP) is one of four community based service networks in Rhode Island established by the Department of Children, Youth and Families in 2009 to provide coordination and direct services for vulnerable families. The FCCP collaborates with parents and youth to address complex family concerns such as: risk for child abuse and neglect, youth with moderate to severe emotional disturbance, children with developmental delays or disabilities and adolescents involved with the Juvenile Justice System.

Family Resources Community Action serves as the Northern Region lead agency subcontracting with NRI Community Services, Gateway Healthcare, Inc. and Comprehensive Community Action Program to provide High Fidelity Wrap-Around Services. Additional subcontracts will be developed with other organizations to provide early childhood and flexible support services.

The Northern RI FCCP collaborates with a comprehensive array of formal and informal resources encompassing food pantries, housing, shelter, adult and youth employment and training, G.E.D., literacy, mental health and substance abuse treatment, early childhood services, healthcare and much more.

FCCP is a family-centered process partnering with families to identify their priority concerns and accessing resources that help achieve positive outcomes. There is no cost to families; FCCP taps a range of funding including state agency contracts, Medicaid, Private Insurance and other flexible resources.

The Northern RI Family Care Community Partnership serves families living in Woonsocket, Cumberland, Lincoln, North Smithfield, Smithfield, Burrillville, Foster, Gloucester, Scituate, Johnston and North Providence.
